ΔΡΑΣΗ :«ΕΝΑΡΜΟΝΙΣΗ ΟΙΚΟΓΕΝΕΙΑΚΗΣ & ΕΠΑΓΓΕΛΜΑΣΙΚΗΣ ΖΩΗΣ» για την περίοδο 2020-2021»
ΑΝΑΚΟΙΝΩΝΕΤΑΙ ότι:
Από σήμερα Δευτέρα 24 Αυγούστου 2020 οι ωφελούμενες/οι γονείς δύνανται να απευθυνθούν για την εγγραφή σε οποιονδήποτε Δομή (Βρεφονηπιακό / Παιδικό Σταθμό – ΚΔΑΠ) του ΚΑΠΑ Δήμου Βέροιας ή στα Γραφεία Διοίκησης του Ν.Π. (τηλ. 2331022178 / 2331022547)
Τα βήματα που πρέπει να ακολουθήσουν είναι τα εξής:
1. Να εκτυπώσουν την «αξία τοποθέτησης» (voucher) για κάθε παιδί, από την ειδική εφαρμογή «οριστικά αποτελέσματα» της Ε.Ε.Τ.Α.Α. (χρησιμοποιώντας τους κωδικούς ΤAXISNET)
2. Να απευθυνθούν, από 24-8-2020 μέχρι και 2-9-2020, σε οποιαδήποτε Δομή της επιλογής τους για την εγγραφή του/των παιδιού/ών τους ή του ατόμου που έχουν την επιμέλεια ή τη συμπαράσταση, δίνοντας την «αξία τοποθέτησης» (voucher)
3. Να συνάψουν /υπογράψουν τυποποιημένη σύμβαση με το Φορέα /Δομή της επιλογής τους. Η σύμβαση υπογράφεται σε δύο (2) αντίτυπα, ένα (1) για την/τον ωφελούμενο/νη, ένα (1) για το φορέα.
4. Να υπογράψουν την εξουσιοδότηση προς την Ε.Ε.Τ.Α.Α. Α.Ε. και να την παραδώσουν στο φορέα που επέλεξαν, προκειμένου αυτός να την υποβάλλει ηλεκτρονικά στην εφαρμογή μαζί με τη Σύμβαση.
5. Να συμπληρώσουν και να υποβάλουν ηλεκτρονικά,την 1η ημέρα παροχής των υπηρεσιών, το Ερωτηματολόγιο Εισόδου, μέσω της ηλεκτρονικής εφαρμογής της Ε.Ε.Τ.Α.Α. («Ειδική εφαρμογή για τα απογραφικά δελτία Εισόδου-Εξόδου»), με τη χρήση των κωδικών-διαπιστευτηρίων της Γ.Γ.Π.Σ.Δ.Δ. (κωδικού taxisnet).
Επισημαίνεται ότι:
α) η εγγραφή θεωρείται έγκυρη και η σύμβαση μεταξύ του Φορέα/Δομής και της/του ωφελούμενης/νου ενεργό, μόνο με τη συμπλήρωση του Ερωτηματολογίου Εισόδου
β) Η «αξία τοποθέτησης» (voucher) διατηρείται σε ισχύ καθ’ όλη τη διάρκεια του σχολικού έτους.
γ) δεν μπορούν να ενεργοποιηθούν vouchers ή να γίνει οποιαδήποτε μεταβολή για τους μήνες Ιούνιο και Ιούλιο του 2021.
